How it works

For babies: download the infant symptom sheet (PDF). Print it and fill it in, then snap a clear photo of the page. No printer? Just type your answers in the email.
For kids 12 months and up: download the child symptom sheet (PDF). Print it and fill it in, then snap a clear photo of the page, or type your answers in the email.
Prefer to do it online? Take our online quiz. It takes a few minutes, and your answers come to our team too.
- Take 3 or 4 photos of the tongue and lip. Tips are below.
- Email them to us with the details in the Send your pre-screen section.
- We’ll email you and let you know whether we think a visit makes sense.
How to take good photos on your phone
It’s easiest with two people: one holds your child, and one takes the pictures.
Before you start
- Wash your hands.
- Find good light. Face a window, or have your helper shine a second phone’s flashlight into the mouth.
- Turn off filters and portrait mode, and don’t zoom. Move the phone closer instead. Tap the screen on the mouth so it focuses.
Lip photo
- Lay your baby on their back (on a bed or across your lap) with the top of their head toward you. This is the same view we use in the exam. An older child can sit and tip their head back a little.
- With a fingertip on each side, gently lift the upper lip up toward the nose.
- Take the photo straight on, so we can see where the lip attaches to the gum.
Tongue photo
- For babies: place a fingertip on each side of the tongue, under it, and gently lift the tongue toward the roof of the mouth. A photo while your baby is crying with the mouth wide open can also show the tongue well.
- For older kids: ask them to open wide and touch the tip of the tongue to the roof of the mouth. Then take one more photo with the tongue stuck out as far as it goes.
- Take the photo from slightly below, so we can see the underside of the tongue.

- A photo of the completed symptom sheet (unless you took the quiz)
Don’t worry if your baby fusses or the photos aren’t perfect. Send your best few. If we need another angle, we’ll ask.
Good to know
- Photos can’t replace an exam. The pre-screen just helps you decide whether the trip makes sense. It isn’t a diagnosis.
- If we think a visit makes sense, the consult and any treatment can happen the same day, so you only make one trip. See our fees and what’s included.
- If you’re worried about your baby’s weight, feeding, or wet diapers right now, call your pediatrician today. Don’t wait on a pre-screen.
